Currently, our enrolments are Closed
Enrolment in a primary health organisation (PHO) is voluntary. Most New Zealanders are however enrolled through their general practice and gain the benefits associated with belonging to a PHO, which can include cheaper doctors’ visits and reduced costs of prescription medicines.
To enrol, you will have to complete and sign a form that requires you to provide some personal details such as name, age, date of birth, address, and ethnicity.

The information collected at enrolment is subject to the Privacy Act 1993 and the Health Information Privacy Code 1994, so the privacy of your information is protected.
All new enrolments require a form of ID this includes a passport or both a driver licence and birth certificate.
There will be a $40.50 fee on registration, (please note your enrolment will NOT be processed
until registration fee has been paid.
